Do I need to fill in my forms before my visit?

Yes, ideally before you arrive. Filling them in ahead of time means our front desk can review everything beforehand and your appointment starts on time instead of you filling out paperwork in the waiting room. If you don’t get to it beforehand, that’s okay too, just come a few minutes early and we’ll get you set up at the front desk.

Is my information kept private?

Yes. Your information is transmitted securely and used only for your dental care and records, we don’t share it beyond what’s needed to process insurance claims on your behalf.

Can I use my phone?

Yes, every form works on your phone, tablet, or computer. Fill it in whenever and wherever is easiest for you.

What if I make a mistake or don’t know an answer?

No problem, leave it blank or make your best guess. We’ll review everything with you at your visit and can fill any gaps together.”
